Implementation of NAVIGATE Coordinated Specialty Care for First Episode Psychosis: the Michigan Experience

Study Objectives
Coordinated specialty care (CSC) is widely accepted as an evidence-based treatment for first episode psychosis (FEP). The NAVIGATE intervention from the Recovery After an Initial Schizophrenia Episode Early Treatment Program (RAISE-ETP) study is a CSC intervention which offers a suite of evidence-based treatments shown to improve engagement and clinical outcomes, especially in those with shorter duration of untreated psychosis (DUP). Coincident with the publication of this study, legislation was passed by the United States Congress in 2014–15 to fund CSC for FEP via a Substance Abuse and Mental Health Services Administration (SAMHSA) block grant set-aside for each state. In Michigan (MI) the management of this grant was delegated to Network180, the community mental health authority in Kent County, with the goal of making CSC more widely available to the 10 million people in MI. Limited research describes the outcomes of implementation of CSC into community practices with no published accounts evaluating the use of the NAVIGATE intervention in a naturalistic setting. We describe the outcomes of NAVIGATE implementation in the state of MI.
MethodsIn 2014, 3 centers in MI were selected and trained to provide NAVIGATE CSC for FEP. In 2016 a 4th center was added, and 2 existing centers were expanded to provide additional access to NAVIGATE. Inclusion: age 18–31, served in 1 of 4 FEP centers in MI. Data collection began in 2015 for basic demographics, global illness (CGI q3 mo), hospital/ED use and work/school (SURF q3 mo) and was expanded in 2016 to include further demographics, diagnosis, DUP, vital signs; and in 2018 for clinical symptoms with the modified Colorado Symptom Inventory (mCSI q6 mo), reported via an online portal. This analysis used data until 12/31/19. Mixed effects models adjusted by age, sex and race were used to account for correlated data within patients.
ResultsN=283 had useable demographic information and were included in the analysis. Age at enrollment was 21.6 ± 3.0 yrs; 74.2% male; 53.4% Caucasian, 34.6% African American; 12.9 ± 1.7 yrs of education (N=195). 18 mo retention was 67% with no difference by sex or race. CGI scores decreased 20% from baseline (BL) to 18 mo (BL=3.5, N=134; 15–18 mo=2.8, N=60). Service utilization via the SURF was measured at BL (N=172) and 18 mo (N=72): psychiatric hospitalizations occurred in 37% at BL and 6% at 18 mo (p<0.01); ER visits occurred in 40% at BL and 13% at 18 mo (p<0.01). 44% were working or in school at BL and 68% at 18 mo (p<0.01). 21% were on antipsychotics (AP) at BL (N=178) and 85% at 18 mo (N=13) with 8% and 54% on long acting injectable-AP at BL and 18 mo, respectively. Limitations include missing data and lack of a control group.
ConclusionThe implementation of the NAVIGATE CSC program for FEP in MI resulted in meaningful clinical improvement for enrollees. Further support could make this evidence-based intervention available to more people with FEP.

Discovering dad: paternal roles, responsibilities, and support needs as defined by fathers of children with complex cardiac conditions perioperatively

Background:
Understanding perceptions of family caregivers’ roles and responsibilities regarding their child with complex cardiac needs has potential to help care teams better support parents. Paternal experience has been under-explored in pediatric cardiac cohorts.
Methods:Ten fathers of children undergoing cardiac surgery completed quantitative surveys on their knowledge needs and preferred format of communication. In face-to-face recorded interviews, they responded to open-ended questions about the definition of being a good father to a child with a complex cardiac condition, perceived paternal responsibilities, personal growth as a parent to a child with a complex heart condition, support needs, and recommendations to medical staff for paternal inclusion. Semantic content analysis was utilised. The study reports strictly followed COnsolidated criteria for REporting Qualitative research guidelines.
Results:The fathers reported high preference for knowledge about the child’s heart condition, communication about the treatment plan, and desire for inclusion in the care of their child. Paternal role was defined thematically as: providing a supportive presence, being there, offering bonded insight, serving as strong provider, and acting as an informed advocate. The fathers revealed that their responsibilities sometimes conflicted as they strove to serve as an emotional and economic stabiliser for their family, while also wanting to be foundationally present for their child perioperatively.
Conclusion:This study provides insight into paternal experience and strategies for paternal inclusion. This summary of the self-defined experience of the fathers of pediatric cardiac patients offers constructive and specific advice for medical teams.

Adaptive community-based biodiversity conservation in Australia's tropical rainforests

In the globally significant Australian tropical rainforests, poor performance of community-based natural resource management (CBNRM) approaches mandated by national policy highlights the importance of the global search for better models. This paper reports on co-research to develop, apply and test the transferability and effectiveness of a new model and tools for CBNRM in biodiversity conservation. Adaptive co-management, designed with specific communities and natural resources, recognized as linked multi-scalar phenomena, is the new face of CBNRM. New tools used to achieve adaptive co-management include a collaborative focal species approach focused on the iconic southern cassowary, scenario analysis, science brokering partnerships, a collaborative habitat investment atlas and institutional brokering. An intermediate-complexity analytical framework was used to test the robustness of these tools and therefore likely transferability. The tools meet multiple relevant standards across three dimensions, namely empowering institutions and individuals, ongoing systematic scientific assessment and securing effective on-ground action. Evaluation of effectiveness using a performance criteria framework identified achievement of many social and environmental outcomes. Effective CBNRM requires multi-scale multi-actor collaborative design, not simply devolution to local-scale governance. Bridging/boundary organizations are important to facilitate the process. Further research into collaborative design of CBNRM structures, functions, tools and processes for biodiversity conservation is recommended.

The language and literacy skills of 21 children (aged 6;6), who were selected for linguistic precocity at age 1;8, are reported here. Verbal abilities remained high, and in contrast to the findings at 4;6 (reported in Crain-Thoreson & Dale, 1992), reading achievement is now at a superior level. Overall, the results are consistent with a two-phase model of reading development, in which the second phase is more closely related to language ability than the first. Phonological awareness, as indexed by a phoneme deletion task, appears to emerge as a consequence, rather than a cause, of early reading. There also appears to be a complex relationship among early interest in reading, instruction, and reading development. Differences in child interest in books and book reading may evoke variation in literacy-relevant experiences.

Chip-on-Board technology (COB), or the currently more often used MCM-L acronym (MultiChip Module - Laminate), has long been touted as the low cost, high density electronics packaging choice of the future. Unfortunately, poor reliability performance in comparison to traditional plastic packaging, has been a trademark of COB/MCM-L. Recent advances in processes and materials for COB/MCM-L are providing a paradigm shift in reliability, and providing cost effective high reliability packaging solutions for COB/MCM-L. This paper examines the history of recent advances in liquid encapsulated COB/MCM-L and provides guidelines for selecting appropriate materials and processes for their successful implementation into cost effective manufacturing.

Herbicide-Crop Rotation for Witchweed Control

Corn, cotton, peanuts, and soybeans in various rotations received six separate herbicidal treatments in a 5-year rotation from 1959 through 1963. Several of the combination treatments were highly effective in controlling witchweed (Striga spp.). Land heavily infested with witchweed required 3 to 4 years of herbicidal treatments before the competition of witchweed was eliminated and normal corn yields were produced. The most effective treatment for the control of witchweed without crop injury was postemergence applications of ½ lb/A 2,4-dichlorophenoxyacetic acid (2,4-D) as needed in continuous corn.
At the end of the 5-year study, all plots were planted to corn, Using corn yields as a criteria, there were no differences in witchweed competition in herbicide treated plots with continuous corn and herbicide treated plots in which corn was rotated with other crops.
